Grade Changes via OnBase System

How do I Change Grades after they have Posted in My Sac State?

Once your grades have been posted in My Sac State and/or the grade submission timeframe has passed, any changes must be handled by submitting the appropriate form to your Department Chair and Registrar’s Office via the OnBase system. Below we go through the steps about how to access the OnBase Grade Change form via My Sac State. Grade Change forms will be routed to the appropriate department for Department Chair approval before being submitted to Registrar's office for final processing. To get started with the Grade Change process, please do the following:

Login to My Sac State
Click on the OnBase Forms Link that displays at top of page.

Click the "OnBase Forms" button/link to open list of Forms for faculty Use.

Click the Grade Change for Faculty link

Click the "Grade Change for Faculty" link to open the OnBase form for submitting a grade change online.
If you accessed the OnBase forms via your My Sac State account you will automatically see another window display called "Grade Change Form". Otherwise you may see a prompt asking you to login, if so, please login with your Sac State username and password.

When ready click on the Form tab to start your request.
On the Form tab under the "Student Information" section, begin by entering the "Student ID" information.
Upon entering the Student ID, the remaining fields under this section (First Name, Middle Name, Last Name, Email, and Phone) will auto-populate.

Verify that the student's information is correct and proceed to the "Course Information" section.
Under "Course Information" fill out all required fields and be sure to select the correct Department as this selection will dictate to what Department Chair the form will be routed for approval prior to processing by Registrar's office.
Review all of the information you entered.
Once you are ready to submit your grade change request, click the Submit button. The form will be routed to the appropriate Department Chair for approval and then to the Registrar's Office for processing.

Please Note: If your grades have been entered but not posted, you can go back and make changes online. (If the approval status has already been set to “Approved”, you will need to change it back to “Not Reviewed” in order to edit the grades on your roster.) The instructions on this page are for grades that have already "POSTED" in My Sac State.
